TITLE: Maternity Group Home Case Worker

PROGRAM/DEPARTMENT: CHD/Children and Families Division

REPORT TO: Safety Zone’s Assistant Program Director

DATE: February 24, 2022

JOB TYPE: Full Time, Non-Exempt

HOURS OF WORK: Monday-Friday 9:00 am to 5:00 pm. Flexible schedule and ability and willingness to work outside of regular business hours are essential.

Salary: 39,000-42,000

POSITION PURPOSE AND OBJECTIVES:

The CHD Safety Zone Maternity Group Home Program is a youth program specifically serving youth, 16 and up to 22 years old experiencing homelessness or at imminent risk of becoming homeless. CHD Maternity Group Home model is a scattered site housing program for youth and young adults and their children with an emphasis of meeting the youth were they are and providing assistance and support that they need to become self-sufficient or obtain the necessary services needed to exist a life on the streets. This Maternity Group Home Program is a federally funded grant issued by the Family and Youth Services Bureau, a federal division of the US Executive Branch under the Administration for Children and Families and the Department of Health and Human Services.

CHD Maternity Group Home case workers will be responsible for managing a caseload of pregnant or parenting youth, and their children by providing both on-site and off-site activities, intensive case management, group and class facilitation, youth assessments, crisis phone counseling, client data collection, documentation, file maintenance, service coordination, and ensuring that the program is facilitated in a manner that meets CHD’s mission. The CHD caseworker will work with the youth at either the CHD office, in the community, and/or at the housing site. The position will be lead with a culturally-competent, youth focused framework that is informed by best and promising practices such as Positive Youth Development, Trauma Informed Interventions, Racial Equity, Motivational Interviewing, YYA Problem-Solving methodology, and Harm Reduction approaches.

ESSENTIAL JOB FUNCTIONS: Includes the following but not limited to.

Intake

  • Completes thorough intakes in a manner that begins to establish comfort and trusting relationships.
  • Assesses youth’s level of need and intent and makes recommendations regarding youth involvement in CHD Maternity Group Home program. Establishes plans with youth, endures follow through, and makes appropriate referrals.

Case Management

  • Provides intensive case management services to assist participants in achieving strength-based/solution-focused goals. The case management plans to reduce barriers and promote permanent housing and self-sufficiency.
  • Provides weekly support to the youth based in the principles of trauma-informed case and positive youth development.
  • Conducts weekly supervision session with the youth to review youth’s progress toward action plan objectives, revise plans, and provide support and accountability.
  • Collaborates with other agencies for service delivery needs, staying current on resources and developments in the area of specialization.
  • Works with parents, legal guardians, co-parents, family members toward reunification, if appropriate.
  • Provides crisis intervention support, phone counseling, and support of the milieu as necessary.
  • Knowledge of and familiar with the various benefit services for parents (WIC,SNAP, CCAP, Medicaid)
  • Work closely with parent support agencies and early childhood education providers to ensure youth and their dependent children have access to parent coaching and early education.
  • Ability to work autonomously off-site housing, as possible.
  • Maintain individual participant files, including monthly logs and outcome reports, to document all aspects of case management, assessments, and achievements for participants. Maintain consistent and thorough individual files of each YA participant.
  • Attend staff meetings and work within a team approach to generate ideas, thoughts, solutions, and actions that support YA ability to thrive.
  • Collect and report program data, including but not limited to, data required for CHD ETO HMIS, and funder’s required data.
  • Work and collaborate with the Coordinated Entry Team including YHDP providers and other YA agencies.
  • Provide crisis support and participate in the development, revision and utilization of crisis prevention and/or safety plans
  • Make necessary referrals and/or connect YA to community resource providers or supports of the following but not limited to; the education system, employment, mental health services, health care providers, nature supports, and so forth.
  • Assist YA in obtaining housing, benefits and health care as needed.
  • Provide service coordination and linkages to the community.

 QUALIFCATIONS:

  • Bachelor’s degree from an accredited educational institution and at least two years of relevant work or lived experience.
  • Experience working with pregnant and parenting youth required
  • The ability to build a rapport, work effectively, and communicate in a culturally responsive manner to the pregnant and parenting youth.
  • Attendance and punctuality are basic job requirements in this job description.
  • Knowledge and experience with data collection and management systems.
  • Knowledge of social services, housing supports, and basic benefit systems.
  • Clean CORI Background Check and Driving Record Screening.
  • Ability to pass certification for CPR, First Aid
  • Valid driver’s license; reliable transportation for business use and vehicle insurance.
  • Skilled to work respectfully with diverse populations including but not limited to LGBT youth and young adults and other vulnerable populations. 
  • Ability to counsel and support youth and young pregnant or parenting teens with the focus of young adult choice in all planning and guidance.
